Submit your group’s Quick Start Awards online by filling out the required information below. Alternatively, you can print out the paper form here and mail it back to Century with your order forms or scan and email it to your Customer Care Specialist. Fill out as many as you need.

Enter your School Name, Group Name, select your Fundraising Professional, enter your Group ID, the student’s first and last name, and their Quick Start Award selection from the options below by entering the awards code.
